The Northeast Clinical Office is the home base for the Hospice of the Valley care teams that serve Scottsdale and the communities of the Northeast Valley, including North and Northeast Phoenix, Fountain Hills, Cave Creek, Carefree, Paradise Valley, Anthem, New River, Salt River Pima-Maricopa Indian Community, Rio Verde, Fort McDowell, Black Canyon City and Crown King. From this office, nurses, physicians, social workers, chaplains and aides coordinate hospice, palliative, dementia and supportive care that is delivered right where patients live.

Being based in the neighborhoods we serve means we are never far away when a family needs us.

This is a clinical office, not a walk-in location. Our care comes to the patient, wherever they call home.
